It is the tenth anniversary of the tragic loss of Space Shuttle Columbia and its crew of seven. The orbiter broke up over Texas during its fiery re-entry, creating a debris field that stretched into Louisiana. The remaining shuttle fleet was grounded for nearly three years while NASA worked to find out why its oldest orbiter failed and how to correct the problem. An investigation showed that a piece of foam insulation fell from the external fuel tank during launch, punching a hole in the leading edge of Columbia's left wing. The damage went undetected until Columbia's destruction nearly two-weeks after the impact:
